Cancelation Request Form

You have the option to FREEZE your membership for one month and it’ll automatically restart after that, no hassle. Just give us a call or use the bot on our main page to set it up.

Before submitting request, please make sure of the following:

This form is being filled out and submitted 5 days in advance of the monthly draft.

This means by the 9th of each month…to be exact at 11:59 PM on the 9th. The request must be for the upcoming month not future months.

Your membership has had a minimum of 2 successful auto drafts.

This is the monthly charge on the 15th of the month with your card on file.

Memberships has a $0 balance.

Monthly drafts will continue to add to balance owed until the balance is at $0. Then requested cancellation can be submitted.

The membership is not coming off a freeze from the month before.
