Cancelation Request Form 

 Before submitting request, please make sure of the following:​​​

•  This form is being filled out and submitted 5 days in advance of the monthly draft. 

This means by the 9th of each month…to be exact at 11:59 PM on the 9th. The request must be for the upcoming month not future months.

 

• Your membership has had a minimum of 2 successful auto drafts.

This is the monthly charge on the 15th of the month with your card on file.

 

• Memberships has a $0 balance. 

Monthly drafts will continue to add to balance owed until the balance is at $0. Then requested cancellation can be submitted.

 

• The membership is not coming off a freeze from the month before.
